Get Your FOKANA Club Card

The FOKANA Club Card membership is exclusively offered to paid members of Malayalee Associations registered with FOKANA.

If not found, instructions to contact their current organization to get it registered with FOKANA
or, instructions to form their own local organization and then get it registered with FOKANA

Note: Personal information gathered through this form will be protected and will not be shared or sold to any third parties.

To apply, members must provide the following: 

1. select their participating organzation that is registered with FOKANA

2. first and last names (including both husband and wife if married);

3. passport-sized photograph of primary account holder; 

4. a telephone number (preferably a cellphone number) to enable text message communication with members; 

5. their residential address. 

6. at least two emergency contact details (phone number and email) of relatives or friends.

7. Card will have a validity of two years (limited time only)

A nominal fee of $10.00 will be charged initially to offset the cost of issuing the card, depending on expenses incurred. The same fee will apply for renewals or replacements. Each FOKANA Club Card will feature an issue date and an expiration date. Upon expiration, the card will automatically renew unless the member notifies FOKANA in advance via email. Members will be informed of the renewal and charged the membership fee accordingly. No refunds will be issued once the card is provided.

Any net proceeds from the program will be donated to a deserving charity, fostering greater participation from the Malayalee community in the FOKANA Club Card initiative.
